Nurseslabs For All Your Nursing Needs 
The Professional Regulations Commission Southern Tagalog (Lucena City) announced the Schedule of Registration for those who passed the July 2011 Nursing Board Examination.
Registration will commence from September 1 to October 7, 2011. PRC is often during their usual office hours.
REQUIREMENTS
- Two (2) pieces passport size colored picture in white background with complete name tag (lastname, first name, middle name) e.g. GAGALAC, ARNEL ROSARIA
- One (1) piece 1×1 colored picture in white background (same shot)
- Community Tax Certificate (Cedula)
- Payment: Registration Fee (Php 600.00)/ Annual Fee (Php 450.00)
- Metered Documentary Stamp (available at PRC offices)
STEPS IN REGISTRATION
- Present NOTICE OF ADMISSION or any VALID ID at Window 8 (for verification of NLE rating) to secure OATH FORM.
- Check your NAME and DATE OF BIRTH from your verification slip, let us know if there is any discrepancy.
- Fill out the oath form properly.
- Pay annual fee/registration fee at Window 17
- Pay metered documentary stamp (in front of Window 17).
- Submit the properly filled out OATH FORM at Window 5.
- Wait to be called at Window 5 for registration proper.
- After registration, receive your claim slip. (check the presence of the following – registration number and date serial number)

- Secure ACTION SHEET from the guard
- Pay at Window 17 (Php 75.00 per copy)
- Submit action sheet, official receipt and photocopied verification slip/claim slip at Window 1 or 2
