Professional Regulation Commission (PRC) posted the registration schedules and venue for the Manila & Pampanga July 2011 Nursing Board Passers. The registration dates will be from September 1 – 15, 2011 and will be held at the VTECH TOWER, Araneta Avenue, Quezon City. Those who fail to register at their scheduled date should register from September 16 onward.
Requirements
- Duly accomplished Oath Form or Panunumpa ng Professional
- Two (2) pieces passport size colored picture in white background with complete name tag (lastname, first name, middle name) e.g. GAGALAC, ARNEL ROSARIA
- One (1) piece 1×1 colored picture in white background (same shot)
- Community Tax Certificate (Cedula) [Should be dated this year 2011]
- Payment: Registration Fee (Php 600.00)/ Annual Fee (Php 450.00)
- Metered Documentary Stamp (available at PRC offices)
- One (1) short brown envelop with name and profession
Expenses
- 1050 for the registration fee
- 21 pesos for the stamp
- 50 pesos for the notary
- Total: 1121
Oath Form
The oath form should be correctly and completely accomplished. They are available at the PRC offices but you can print one in duplex and using a 8.5×14 in paper.

Procedures
The following are procedures written in the Oath Form. Changes can be made by the PRC during the actual registration period, so follow their procedures.
- Accomplish this Oath Form
- Pay for two sets of metered documentary stamps (one stamped on this form and one for the Registration Certificate) at the Customer Service Center
- Present duly accomplished oath form (where your profession belongs, for verification of records and assessment at Window 14, 19, 28 or at PRC service Center.
- Pay prescribed fee at the Cashier
- Sign in the Registry Book and receive your claim slip at Windows 71, 20, 27
- Claim your professional ID as scheduled. Refer to your claim slip for further instructions.

- Take the MRT and get off at Quezon Avenue Station
- Take a jeepney to Sto. Domingo Church
- Take a tricycle from Sto. Domingo. The tricycle fare is about 25 pesos and can accommodate up to 3 people.

- Take the MRT and get off at Quezon Avenue Station
- Take a jeepney to G. Araneta Avenue and get off at Starbucks
- Cross Araneta Avenue and turn right; V-Tech Tower is a 15-minute walk from the intersection of G. Araneta Avenue and Quezon Avenue. Your landmark is a Shell gas station.
Coming from Manila
- Take a jeepney headed for Fairview
- Get off at G. Araneta Avenue
- Cross Quezon Avenue
- Continue walking down G. Araneta Avenue until you get to a Shell gas station (about a 15-minute walk).
Coming from Monumento
- Take the bus and get off at Quezon Avenue Station
- Take a jeepney to Sto. Domingo Church

- Take a Quiapo jeepney to Sto. Domingo Church

- Take a Quiapo jeepney to G. Araneta Avenue and get off at Starbucks
- Cross G. Araneta Avenue and turn right; V-Tech Tower is a 15 minute walk from the intersection of G. Araneta Avenue and Quezon Avenue. Your landmark is a Shell gas station.
Via MRT
- Get off at the Quezon Avenue Station. Look around for the 7-Eleven near the station.
- There should be an overpass near that 7-Eleven. Go up that and turn left at the second flight of stairs.
- A jeepney station station should be in there. Ride any jeep there and ask to be dropped off at the corner of G. Araneta Avenue.
- You’ll know you’re near if you see the Sanctuarium (it’s that big church that has a dome-shaped ceiling).
- Now get off the jeep.
- Go along G. Araneta Avenue in the direction opposite the Sanctuarium until you see a Shell gas station.
- Pass through the pedestrian lane to get to it.
Via LRT (Line 1)
Get off at the 5th Avenue Station. There should be a convenience store nearby with jeeps waiting a little far off. Ride one and ask to be dropped off at the Shell gas station near Quezon Avenue.
Alternatively, get off at the Sta. Mesa station. Ride any jeep to Quiapo and ask to be dropped off at the Jollibee near G. Araneta Avenue. Cross the overpass and walk along G. Araneta Avenue in the direction opposite the Sanctuarium until you see a Shell gas station.
