The Nurse Licensure Examination is a 500-item multiple choice exam to test the basic nursing level of competency of aspiring nurses. It is held every July and December annually in various public schools throughout the Philippines.
The following contains a list of requirements that are needed for your application for the Philippine Nursing Licensure Examination.
- Duly accomplished PRC Nurses’ Application Form (NAF) [Download it here]
- NSO issued Birth Certificate (original and photocopy)
- NSO issued Marriage Contract for female married applicants (original and photocopy)
- Transcript of Records (TOR) that includes the following:
- Degree/Course
- Exact date of graduation
- Special Order/exemption/accreditation/deregulation status
- Scanned picture and;
- the remark: “FOR BOARD EXAMINATION PURPOSES ONLY”
- Notarized Operating Room and Delivery Room Cases form
- Notarized Summary of Related Learning Experience
- Examination fees:
- Baccalaureate Degree: PhP 900
- Non-baccalaureate Degree: PhP 600
- Removal: PhP 450
- Four (4) pieces passport-sized pictures in white background with complete name tag (RIZAL, JOSE P.)
- One (1) piece window mailing envelope with metered postage stamp
- Current Community Tax Certificate (Cedula)
- Other specific requirements (if there’s any) as required by the PRC.
Be sure to complete your requirements for the Philippine Nurse Licensure Examination at least three days before the examination day to avoid any mishaps.
