2. Resume with colored 2×2 picture
3. TOR
4. Diploma
5. Board Rating (breakdown of grades)
6. Board Certificate
7. Birth Certificate
8. PRC ID (with signature at the back)

Note: *Applicants should not be more than 35years old.
* Follow ups of application is every MONDAY.
*Applicants must be at least 5 feet in height.
